TBI (Traumatic Brain Injury)

After a concussion, service members will be evaluated by their Primary Care Provider.  If they remain symptomatic after the acute stage of recovery (two weeks up to one month), they may benefit from evaluation and treatment at the Intrepid Spirit Center, Womack Army Medical Center. They should talk with their primary care manager if they are unsure about the symptoms they are experiencing. All soldiers who have received a referral from their Primary Care Provider may schedule an appointment at the Intrepid Spirit by calling 910-907-7777. Appointments are available Monday-Friday, 8:00 a.m.-4:00 p.m.  Currently the Intrepid Spirit is only able to provide services for service members.  Family Members and Retirees are referred to a specialty provider off post.

Concussion Care Clinic

The Concussion Care Clinic (CCC), Department of Brain Injury Medicine, is responsible for the evaluation and treatment of patients who have sustained a concussion or traumatic brain injury (TBI). This includes management of patients with persistent post-concussive symptoms (PPCS). For patients with PPCS, clinic care providers perform a detailed history and physical examination in order to better identify the cause of each persistent symptom, and customize a treatment program. Clinicians within the CCC coordinate and supervise any rehabilitation services deemed necessary, and ultimately make recommendations regarding physical limitations and fitness for duty of active duty patients.

Multidisciplinary Team

The Intrepid Spirit utilizes a holistic, multidisciplinary approach to better determine the cause of persistent post concussive symptoms (PPCS). They provide comprehensive treatment for functional deficits associated with concussion/ traumatic brain injury.  Team members meet regularly to discuss individualized patient goals and progress, making changes as needed to ensure maximum recovery.

Services include the following:

  • Neuro-Optometry directs vision rehab, which can improve symptoms - such as headaches, perceptual deficits, and balance - as well as service members' activities of daily living, such as shooting, reading, and routine soldier tasks.
  • Neuropsychologists evaluate through testing the nature and extent of impairment following TBI and plan strategies to compensate for the injury.
  • Occupational Therapists help service members who have sustained a TBI to develop, recover or maintain their ability to perform tasks of daily living.
  • Physical Therapists evaluate and treat vestibular and balance disorders commonly associated with head injury.
  • Speech and Language Pathologists assist service members who have speech, language, and cognitive (attention, memory, problem solving) problems to regain optimal communication.
  • Social Workers/Psychologists provide evaluation, education, supportive services, and referrals for soldiers and family members to decrease psychiatric distress and promote wellness strategies.

Intensive Outpatient Program

The clinic also offers an Intensive Outpatient Program, a four-week program for service members.  During the program, the clinic team works with the SM and their families to create a tailored treatment plan that focuses on creating a plan of action to increase resilience and well-being as well as learning the skills they need to self-manage their symptoms in the long term. The mission of the IOP / PAR program is to return the SM to his/her parent unit as quickly as possible, and ensure that, if cleared for duty, s/he is able to safely perform basic level FORSCOM skill requirements to meet the standards that unit leadership requires.

The CCC also partners with local and national Defense and Veterans Brain Injury Center personnel who coordinate educational sessions on topics such as concussion, TBI, and PPCS for other medical providers and various units/agencies of the Fort Bragg community.  In addition, support services are available through a Regional Support Specialist who assists service members and families with ensuring continuity of care and access to services during transitions from active duty to civilian life and/or PCS moves.

Army Hearing Program

Mission

To promote army readiness, quality of life and reduce incidence of noise-induced hearing loss through a state-of-the-art hearing loss prevention program across Fort Bragg installation.

Scope of Services

The Army Hearing Program (AHP) per provides information on hearing, hearing loss prevention, hearing protection, DOEHRS-HC, hearing monitoring, and reducing noise hazards. The AHP consists of four components:
·         Hearing readiness is designed to ensure Soldiers have the required hearing capability to perform their job-specific duties, as well as the appropriate and properly-fitted hearing protection devices for their mission. The primary purpose of hearing readiness is to identify early changes in hearing and provide education, individual counseling, and hearing protection to prevent damage to hearing. A key element of hearing readiness is monitoring audiometry, which is conducted with the current DOD-approved microprocessor audiometer system comprised of the Defense Occupational and Environmental Health Readiness System-Hearing Conservation (DOEHRS–HC) software application, the current DOD-approved audiometer, and the DOEHRS–HC Data Repository (DR) Web-based Application. All Soldiers are required to undergo an annual DOEHRS–HC audiogram. Monitoring audiometry data is made available through MEDPROS, thus providing the capability for monitoring and tracking of hearing for both individual and unit-level hearing readiness.

·         Clinical hearing services are intended to determine a Soldier’s auditory fitness-for-duty (FFD) by determining the degree and cause of hearing loss and measuring a Soldier’s hearing ability against a performance or medical standard. Clinical hearing services for at risk DA Civilians who are exposed to hazardous noise are intended to determine work-relatedness of hearing loss. After degree and cause have been determined, treatment and management plans can be initiated. Treatment plans may include medical or surgical intervention, prescription of personal hearing aids, assistive listening devices, aural rehabilitation, tinnitus management, new hearing protection or monitoring of the condition for changes.

·         Operational hearing services focus on preventing or mitigating NIHL during military operations while maintaining or facilitating the Soldiers’ ability to communicate. Operational hearing services focus on noise assessment and abatement strategies, improved communication, and the use of specialized hearing protection devices, such as non-linear and linear dual-function earplugs designed to reduce the impact of noise and NIHL on military operations. Tactical Communications and Protective Systems (TCAPS) are designed to amplify low-volume sounds and facilitate face-to-face and radio communications while simultaneously protecting the ear from hazardous impulse noise (such as, weapons fire). These types of devices protect hearing in both operational training and combat environments while allowing Soldiers to maintain their ability to hear and communicate, especially in dismounted operations.

Hearing conservation - For many years, the AHP served as the flagship for the prevention of noise-induced hearing loss. The hearing conservation component of the AHP is industrial-focused and is instrumental in preventing NIHL in primarily fixed-facility work environments. HC efforts are conducted in support of both the DA Civilian workforce and Soldiers working in industrial-based settings. While HC requirements continue to exist in their previous form and function, they now serve in concert with additional AHP components to provide comprehensive hearing loss prevention and hearing health promotion program.

ARE YOU A CANDIDATE?

co-morbidities such as Type 2 diabetes, hypertension or hyperlipidemia.

If you meet these criteria, please contact your PCM for an appointment to discuss the possibility for surgery and referral.

Types of Procedures Offered

We offer four types of weight loss procedures at Womack Army Medical Center:

  • Sleeve Gastrectomy
  • Roux-en-Y Gastric Bypass
  • Gastric Banding
  • Duodenal Switch

Sleeve Gastrectomy
The Vertical Sleeve Gastrectomy is a restrictive form of weight loss surgery where approximately 85% of the stomach is removed. The part of the stomach that is cut away is responsible for producing the hormone, grehlin which is partly responsible for stimulating hunger. This surgery is NOT reversible.
This surgery is not approved for patients wishing to be seen by surgeons outside of a military treatment facility.

Roux-en-Y Gastric Bypass
The Roux-en-Y procedure involves creating a small stomach pouch and attaching it directly to the small intestine, bypassing a large part of the stomach and duodenum. This new stomach pouch is approximately 30cc (the cap that comes with Nyquil) which will hold only small amounts of food. The food will then bypass the duodenum which is responsible for most of the fat absorption.

Lap Band (Gastric Banding)
Is a procedure that places an adjustable band around the top portion of the stomach to create a smaller pouch. A port is placed beneath the skin to allow a certified bariatric doctor access to either inflate or deflate the band. This weight loss procedure works purely by restriction. While lap banding is a reversible procedure, it is not an easy one.

Duodenal Switch
In this procedure, a larger portion of the stomach is left intact, including the pyloric valve that regulates the release of contents from the stomach into the small intestine. The duodenum is divided near this valve, and the small intestine divided as well. The portion of the small intestine connected to large intestine is attached to the short duodenal segment next to the stomach. The remaining segment of the duodenum connected to the pancreas and gallbladder is attached to this limb closer to the large intestine. Where contents from these two segments mix is called the common channel, which dumps into the large intestine.
